FACTS CONCERNING NEEDLESTICK INJURIES AT HEALTH CARE FACILITIES Numerous medical accidents pass reported. Incidents involving syringe needles required special attention. According to a survey conducted by the Ministry of Health, Labor and Welfare, the number of reported of needle-stick injures cases in 1996 was 3,867. Among them, cases resulting in virus infection were to 3,386.Although official data is not available in Japan, needle-stick injures in the United States have numbered from 800,000~1,000,000 cases. From the previously collected data in Japan, including hospitals, clinics and dental offices, only 174 hospitals among 150,000 health care facilities have officially been reported. As the above offices report indicated, 3,867 cases of needle-stick injures have been identified, among which 3,386 (90%) were related to virus infection. * Actual cases may total to approximately 400,000 - 500,000 in Japan, from our estimation. Prevention of needle stick incidents is not only the hospitalfs concern but has to do with the well being of our community Patients experiences needle stick injuries at hospital specialized in the treatment of AIDS (Mainichi Shinbun, April 19, 2000)Among the nationwide hospitals specialized in the treatment of AIDS, there were a number of cases in which doctors and nurses inadvertently pricked a needle point used after treating AIDS patient or hepatitis patients. From 1996 to 1998, total of 15,119 cases were reported, among which 28 cases consisted of instance contacting hepatitis B (HBV). According to a study by AIDS research division of the Health and Welfare Ministry. This research division reported that 88 cases involved needles treated in HIV patients. 1,862 cases were HBV patients while 7,708 cases were HCV patients. Although none of the doctors and nurses contacted with HIV, 28 have transmitted HCV. Highest numbers were cases occurred in hospital room (4,557), followed by nurse station and corridor (1,920) and surgical department (1,857). It also included incident during medical treatment (3,665), putting a needle cap (3,001) and during the period between treatment and disposal (2,476). gAmong the health care sectors in Japan, we tend to conclude the cause of incident to a as ecarelessness,f and that is might answer why such accidents do not reduce or minimize.h says a doctor who asks not to be named. gI suggest introducing a syringe needle with protection cover or cap which is to cover the needled after uses.h It is our priority to help protect the environment while doctors strive to save the lives of the patients Treatment for needle-stick Injuries What would you do when you accidentally prick yourself with a needle treated on chronic hepatitis B or C patient? The needlepoint is tainted with hepatitis B, C virus and the injured personfs (such as doctor and nurse) blood test has not yet been conducted. In what follow, we will explain the necessary procedure to be taken and some useful measures to cope under such situation. HBV HBV (hepatitis B virus) Immediate Measures Wash with running water and extract the blood from the needle stick area. This is an essential step and be sure to inform the doctor/nursefs colleague of relatives about the accident Treatment at initial stage1. Cheak the patientfs hepatitis B e antigen (HbeAg) or hepatitis B e antibody (HbeAb) 2. Cheak the injured personfs HB s antigen or HB s antibody This should be done if the patient has past infection of HBV or recognized as carrier of the virus. If the injured personfs HB s antibody is tested positive (16 fold with PHA Method), or carry B s antigen his will not be the case. When the injured person does not carry an antibody: 1. Provide HB s antibody containing immune globulin (HBIG) –within 48 hours at the latest. 2. Deliver additional HB Vaccine èIf the source if contagion is diagnosed as HB s antigen positive after giving HBIG, development of hepatitis in the injured person may not be prevented. Incase of needlestick injury, it is preferable to impart active immunization within 4 months period. 3. General hepatic function study should also be conducted. When requesting for the examination for HB s antigen and antibody, one should urge the examiner to promptly return the testing result. (Treatment will be determined according to the result) Make sure HBIG is in stock. Follow up treatmentProvide the second time of HB Vaccine after one month and the third after three months. We recommend patients to take hepatic function test at least for the duration of 6 months. Other remarks Hepatitis B can be effectively prevented by taking HB vaccine in advance. Q) Is it necessary to provide interferon right way? A) No. Interferon is not a virus neutralizing antibody. Delivering interferon immediately after needle-stick injury seems less effective for preventing hepatitis. It also has side effects. (it works well, in case of acute hepatitis C) Treatment at initial stage Check the injured (such as doctors, and nurses) patientfs HCV antibody. Check the patientfs (who is under general hepatic function test) level of HCV antibody. Has the RNA examination for virus infection been conducted? HCV Ab positive patients include those who did not become a carrier (as with those with past infections), despite having acute hepatitis C. This can be determined by the strength level of HVC Ab (high; 2x10 - 12 or 13, low; 2x10 - 7). Patients may have past infection if the level id low. It depends on the PCR method delivered on a patient to identify RNA virus. Follow up treatment

A) No, it only takes 2, 3 seconds. Insert the used syringe needle into the slot. Push the plate until the float reaches the bottom and remove when the process is completed. Q) Why do we keep the needle point on the syringe during the process? A) It prevents the residue of blood and chemical left in the syringe from spilling out. A) Disposal standards and procedures vary depending on governments and hospitals policies. The indicator lamp on the device will light to inform of completion. It can also be operated with the AC power connected. (12 hours is needed for full recharge, when left for a long time).
